Proof of Address: ( This can be a rental receipt or mortgage agreement, water, gas or electric bill. We CAN NOT accept phone or cable TV bills, driver's license, checks, insurance, or envelopes.)

Certified Birth Certificate: (We CAN NOT ERROR MSGaccept a hospital record of birth.) The birth certificate must have a state seal or a state registration number on it. Kindergarten students must be (5) years old on or before September 1, of the current year. First graders must be (6) years old on or before September 1, of the current year.

Florida Certificate of of Immunization: (HRS-680) (BLUE SHOT CARD) We CAN NOT accept military shot records. (The Immunization Clinic at the Navy Hospital will transfer the information onto the proper form for you.) All kindergarten, first, and second grade students must have the hepatitis shots for entry into school. All students must have 2 MMR's.
